Amy Rose, the home visitor, works with Jacob, a 4 1/2 year old child and his mother, Martha, who is currently in recovery from substance abuse. The parent-child activity is directed toward relieving some of Jacob’s anxiety about an impending move to another school. Amy Rose also works with Martha to update the Individual Family Support Plan.
